Elder/Disabled Outreach & Caregiver Support
Suffield Community Aid Outreach Program provides voluntary social work services to senior and disabled residents of Suffield. Information and referral services are also available to any Suffield resident who is caring for a family member outside of Suffield. The goal of the outreach program is to link people with the services and programs they require to remain independent as long as possible. If that is not feasible, we are able to help explore necessary alternatives.
Outreach Social Workers are available to meet with the resident and/or their family in their home or at the office. Telephone consultation is also available. We are able to educate residents regarding the myriad of state, federal and local programs and services that can benefit seniors and persons with disabilities.
We are able to conduct social, physical and financial assessments, make referrals, assist with applications and advocate for residents in need of support services. Some of the types of issues we can help with include: housing, financial, insurance, socialization, health, nutrition, transportation, etc.
The staff works closely with family members, caregivers and local, state and federal agencies to ensure that all resources are leveraged for the benefit of the person seeking assistance.
There is no charge for Outreach Program services.
